If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 8 weeks before you travel, longer if complex or long term travel is planned. Not all travel vaccinations are included in the services provided by the NHS and may incur a charge. Some vaccines may not be avaliable at the surgery and the nurse may advise you to go to a Private Travel Clinic for those vaccines. The surgery is NOT a Yellow Fever Centre and therefore cannot give Yellow Fever Vaccinations or Certificates.

To help us offer the appropriate advice, please fill out the online form and submit to reception when booking your appointment. Reception will offer you an appointment 2 weeks following the submission of the Questionnaire. This form must be received before an appointment will be booked.
