If you are more than six weeks pregnant you can now book your maternity care directly with Poole Hospital's maternity services by filling in a self-referral form. 

The form asks you questions about yourself, your family and your medical background.  Please answer as fully as possible, as this will enable us to ensure that you receive the most appropriate care.

When you are about 10 weeks pregnant, a midwife will either call you, or text you, to arrange a booking appointment, which usually takes place in the 10th or 11th week of pregnancy.  At this appointment you will also receive details of your 12-week scan.
